What happens to my patient record (”journal”) if I change healthcare center?

When you receive care, the healthcare staff records the information in your patient record (in Swedish”journal”).

If you switch to a new healthcare center, the staff there may sometimes lack access to older patient record entries. For them to see these records, you need to give your consent. This is called providing consent (in Swedish “att ge sitt samtycke”). The staff at the clinic can help you with this.
